We were also awarded one of the most Ethical companies Awards in DFW for 2010.  This position will be providing patient care in the Greenville/Bonham area.

The primary function of this position is to provide coordinated care in the home or facilities to patients of all age groups and perform psychosocial assessments, analysis, counseling and referrals to meet the needs of the patient and family.  Assists the physician and other team members in understanding the significant social and emotional factors related to the health problems, participates in the development of the plan of treatment, prepares clinical and progress notes, works with the family, utilizes appropriate community resources, participates in discharge planning and in-service programs and acts as a consultant to other office personnel.

QUALIFICATIONS:

Bachelors in Social Work from a State accredited program. Masters Degree preferred. 
Must have at least one year of social work experience in a health care setting. Hospice experience a plus. 
Must be board licensed in the State of Texas. 
Must possess excellent written and verbal communication skills. 
Must have dependable transportation and a valid driver’s license and current auto liability insurance. 
Must have basic computer skills. 
Ability to work in the Greenville/Bonham and surrounding areas on a daily basis.
