The Texas State Legislature has recently passed a new law that requires
all new students (U.S. or International) who will live in on-campus
housing to have the Bacterial Meningitis Vaccination. This only applies
to new students planning to live in on-campus housing (residence halls).

All new Texas A&M University-Commerce students who plan to live on
campus after January 1, 2010 must provide the following before moving
into campus housing:

*	Certification from a physician (see attached form) evidencing
that the student has been vaccinated at least ten (10) days prior to
moving into the residence halls; or
*	An affidavit or a certificate from a physician stating that the
vaccination would be injurious to the health and well being of the
student or stating that the vaccination has been declined for (personal,
religious, other) reasons objecting to the vaccination. The approved
form for an exemption can be found here:
https://webds.dshs.state.tx.us/immco/affidavit.shtm
<https://webds.dshs.state.tx.us/immco/affidavit.shtm> . If students are
over the age of 18, they can complete the form themselves, listing
themselves as the parent or legal guardian.

New international students who plan to live in campus housing have two
options to satisfy the new law:

*	Receive the Bacterial Meningitis vaccination in your home
country prior to departure (at least 10 days before moving into the
residence halls), complete the attached Evidence of Vaccination Against
Bacterial Meningitis and signed by your Health Practitioner, and submit
this form to the Department of Residential Living and Learning in person
in Halladay Student Services building Room 100.

OR

*	If you object to having the vaccination you will need to order
the above affidavit at
https://webds.dshs.state.tx.us/immco/affidavit.shtm
<https://webds.dshs.state.tx.us/immco/affidavit.shtm> IMMEDIATELY and
have the form mailed to the International Student Services office at PO
Box 3100, Commerce TX, 75429. The ISSO is not responsible if the form
is not delivered in a timely manner or is lost by the postal service.

If you wait until you arrive in the U.S. to have the vaccination you
will not be eligible to move into the residence until 10 days after the
vaccination. Also, the vaccination is very costly and you may not have
medical insurance or it may not be covered.

If you wait to order the exemption form when you arrive in the U.S. or
have it mailed to your home country it may take the form up to two weeks
or longer to be mailed and it must be on file with the Department of
Residential Living and Learning prior to moving in.

Any student planning to live in the residence halls who fails to submit
the required documentation in the required timeframe will be responsible
for their own housing expenses.

Any questions may be directed to the Department of Residential Living
and Learning at 903-886-5797 or email residencelife at tamu-commerce.edu.
