Texas State Legislature has recently passed a new law that requires <u>all new
students (U.S. or International) who will live in on-campus housing to have the
Bacterial Meningitis Vaccination.</u></span><span style='font-size:14.0pt;
font-family:"Arial","sans-serif";color:red'> </span><u><span
style='font-size:16.0pt;font-family:"Arial","sans-serif";color:red;background:
yellow;mso-highlight:yellow'>This only applies to new students planning to live
in on-campus housing (residence halls).</span></u><span style='font-size:14.0pt;
font-family:"Arial","sans-serif";color:red'> <o:p></o:p></span></p>
<p class=MsoNormal><b><u><span style='font-family:"Arial","sans-serif";
color:red'><o:p><span style='text-decoration:none'> </span></o:p></span></u></b></p>
<p class=MsoNormal><b><u><span style='font-family:"Arial","sans-serif";
color:red'>All new Texas A&M University-Commerce students who plan to live
on campus after January 1, 2010 must provide the following before moving into
campus housing:<o:p></o:p></span></u></b></p>
<p class=MsoNormal><span style='font-family:"Arial","sans-serif";color:navy'><o:p> </o:p></span></p>
<ul style='margin-top:0in' type=disc>
<li class=MsoNormal style='color:navy;mso-list:l4 level1 lfo3'><span
style='font-family:"Arial","sans-serif"'>Certification from a physician </span><span
style='font-family:"Arial","sans-serif";color:#1F497D'>(see attached form)
</span><span style='font-family:"Arial","sans-serif"'>evidencing that the
student has been vaccinated at least ten (10) days prior to moving into
the residence halls; or<o:p></o:p></span></li>
<li class=MsoNormal style='color:navy;mso-list:l4 level1 lfo3'><span
style='font-family:"Arial","sans-serif"'>An affidavit or a certificate
from a physician stating that the vaccination would be injurious to the
health and well being of the student or stating that the vaccination has
been declined for (personal, religious, other) reasons objecting to the
vaccination. The approved form for an exemption can be found here: <a

If students are over the age of 18, they can complete the form themselves,
listing themselves as the parent or legal guardian.<o:p></o:p></span></li>

<p class=MsoNormal><b><u><span style='font-family:"Arial","sans-serif";
color:red'>New international students who plan to live in campus housing have
two options to satisfy the new law:<o:p></o:p></span></u></b></p>

style='font-family:"Arial","sans-serif"'>Receive the Bacterial Meningitis
vaccination in your home country prior to departure (at least 10 days
before moving into the residence halls), complete the attached <i>Evidence
of Vaccination Against Bacterial Meningitis</i> and signed by your Health Practitioner,
and submit this form to the Department of Residential Living and Learning <u>in
person</u> in Halladay Student Services building Room 100.<o:p></o:p></span></b></li>
</ul>

color:navy'>OR<o:p></o:p></span></b></p>

75429. <u>The ISSO is not responsible if the form is not delivered

U.S. to have the vaccination you will not be eligible to move into the
residence until 10 days after the vaccination. Also, the vaccination is
very costly and you may not have medical insurance or it may not be covered.</span></b><b><span

the form up to two weeks or longer to be mailed and it must be on file with the
