The Health and Human Performance Needs Assessment Committee made up of Public Health students from A&M University-Commerce need your input as part of their Program Planning class. Public input can be provided through an online survey until April 21st 2020

The survey is brief and should only take 10 to 15 minutes to complete, and asks about your perceptions of health, health issues and challenges within the city. Survey respondents remain anonymous. The Needs Assessment Committee will use the results of the survey along with other information to identify the most pressing health issues that can be addressed through community action.
