[Intl-stu] Submit insurance exemption form now if eligible

A new waiver form has been created for students who are eligible for an
exemption from the mandatory insurance enrollment in the A&M System
insurance provider that began this spring semester. ALL students THAT
ARE ELIGIBLE FOR AN INSURANCE EXEMPTION are required to submit the
insurance waiver form for each semester they are eligible for the
exemption. Failure to submit the form PRIOR to the start of the
semester you are exempted may result in your student account being
charged for the insurance premium.
The form may be found at
http://www.tamu-commerce.edu/international/insurance_exemption.asp or at
the ISSO in Halladay 104. The deadline to submit this form is Friday,
August 26 BUT I would STRONGLY recommend submitting the form and
required documents before the end of the spring semester so we can
confirm your information.
I am working with the Bursar's office to make sure we do not have any
more technically problems with the insurance charge. Once I finalize
the new process I will notify students via email to the list serve of
when they can expect the insurance charge to show on their account.
If you meet one of the exemptions below please take a moment before the
end of this semester to submit the form and provide the required
documents. Please DO NOT submit the Insurance Exemption form if you DO
NOT meet one of the exemptions below. Exemptions will not be considered
until the required information mentioned below is submitted.
To remind you, all F-1 and J-1 international students are automatically
enrolled in the A&M System insurance provider, AIP, unless they meet one
of the exemptions below:
* Students with full-time graduate assistantship and enrollment
in the GA insurance through the Human Resource (HR) department. If you
have a full-time graduate assistantship for the fall 2011 semester and
have enrolled in the GA insurance plan with HR please complete the
Insurance Exemption form at
http://www.tamu-commerce.edu/international/insurance.asp and have your
department email confirmation of your GA for the fall semester and have
HR email confirmation of your GA insurance to
intl_stu at tamu-commerce.edu.
* Students sponsored by the United States Government, a foreign
government recognized by the United States of America, or certain
international, government sponsored or non-governmental organizations
that includes health insurance that meets the university minimum
coverage requirements as stated at
http://www.tamu-commerce.edu/international/insurance_minimum.asp.
Please contact the ISSO to discuss your scholarship.
* Students with a spouse employed IN THE U.S. that are covered
under their spouses health insurance plan. Please note: It is YOUR
responsibility to provide proof that the insurance meets the university
minimum coverage requirements stated at
http://www.tamu-commerce.edu/international/insurance_minimum.asp. Your
insurance provider will need to either email confirmation that your
insurance coverage meets the minimum coverage requirements (see above
link) to intl_stu at tamu-commerce.edu <mailto:intl_stu at tamu-commerce.edu>
or a letter stating your insurance coverage meets the minimum coverage
requirements (see above link). A simple statement of coverage will not
be sufficient. The ISSO must have confirmation that the coverage
specifically meets the university minimum requirements.

submit the exemption form, and provide the appropriate information for
your exemption to intl_stu at tamu-commerce.edu or in person at Halladay
104.
