*         Issues to consider when completing the open-ended comments:

o   Please provide specific illustration of how our services impacted you as a faculty member.  Offer specific reasons for what you feel needs to be kept, changed or enhanced.

o   While comments regarding what needs to change may come more readily, keep in mind that it is just as helpful to remind us about what went well.
