Hello:

Thank you to all you who have completed the short survey for the Faulty 
Senate.   We would like to give you one more chance to participate if you 
have not already done so.  Final tabulations will be next Monday. Try to 
respond before then.  We have had over 100 responses.

If you have completed the survey please disregard this email.  If you have 
not completed the form please take a minute or two then forward the results 
to me. 

NOTE: When completing this survey, DO NOT use the "reply" button but rather
use the "forward" button and then forward your completed survey to my email

not be received in my office.
