[Vpaaetal] Call for FY 2006 Course Fee Requests - Action Required by Decembe r 2, 2004

	This is a call for Course Fee Requests for FY 2006.  Your Course Fee
Request forms will be due by no later than Thursday, December 2, 2004, at
5:00 p.m.

	Attached is a form you may use to request course fees.  Also, please
read the instructions document attached before completing.  Complete the
form and return to me at Jan_Swart at tamu-commerce.edu.  You can return as
many forms as you require.  Please give each individual form a unique name.


	The form contains protected cells and cells that will automatically
calculate the fee requests.  If the amount the spreadsheet calculates for
you is not the amount  you want to request, you can complete the second page
with what fee you would like to charge and justification for the fee.
