<P><FONT COLOR="#000080" SIZE=2 FACE="@Arial Unicode MS">Attached is a form you may use to request course fees. Also, please read the instructions document attached before completing. Complete the form and return to me at Jan_Swart@tamu-commerce.edu. You can return as many forms as you require. Please give each individual form a unique name. </FONT></P>
<P><FONT COLOR="#000080" SIZE=2 FACE="@Arial Unicode MS">The form contains protected cells and cells that will automatically calculate the fee requests. If the amount the spreadsheet calculates for you is not the amount you want to request, you can complete the second page with what fee you would like to charge and justification for the fee.</FONT></P>
