I'm no authority, but I'd lean toward agreeing with Jeff.  I looked at their website, and there is a caution about copying and scanning the blue form because of the word VOID showing up.  Well, we have blue forms, red forms, orange forms, green forms, purple forms, and just about every other color form we call TRANSCRIPTS which we scan into an electronic filing system, and we take them as our good copy and we AVOID the VOID pretty successfully.

I'd wonder just WHY Wes Hodgson says it CANNOT be copied.  If it's just because of the script-safe paper, then maybe he needs to take a field trip to a few campuses to see how the work gets done.

I would recommend having your own general counsel review this.  A scanned copy of a document is equivalent to the original, and they do not have authority over our record-keeping practices.

Good Afternoon!

During the meningitis update session at TACRAO, we were informed that a student could request the meningitis record to be transferred to another school.  Most schools scan this form and destroy the blue exemption form with the serial number on it.  We were informed that we should not be destroying the blue exemption form.  I asked if we could copy the blue exemption form and hand the original back to the student so they could keep up with the document.  This is a similar process we follow for immunization records from the state.  We keep a copy and not the original record.

I was asked to share the answer once I was informed.  Wes Hodgson from DSHS called me and informed me today that the blue form CANNOT be copied.  This includes scanning.  This was information provided to him by his legal counsel.
