Social Worker, LMSW or LCSW

Home Hospice of Grayson County is looking for a Social Worker, LMSW or LCSW.

The oldest hospice in Grayson, Cooke and Fannin counties we are the only non-profit accredited hospice providing care with an interdisciplinary team for chronically-ill and terminally ill patients

This is a full time position with benefits, with regular schedule Monday-Friday, 8:30 am to 5 pm plus on-call rotation for nights and weekends.

The social worker is a core member of the Home Hospice interdisciplinary team (IDT) and provides social work service to patients in accordance with the philosophy, policies and processes of Home Hospice and as required by federal and state statues and the Medicare Conditions of participation.

A minimum of one year of health care social work experience is required for LBSW. Hospice experience is a plus. LMSW or LCSW preferred.

Service area is Cooke, Grayson, and Fannin Counties and willingness to work in Agency's entire service area is required.
