[Intl-stu] Students Graduating in Spring 2023!!

[International Students]

Greetings Students!

We hope this email finds you well.

Attached to this email, you will find the refund request form for students who will be graduating in May.

Please note that the amount has been pre-determined by Academic Health Plans and will be a refund of $513 (calculated by a percentage of the annual premium).  After you have completed the refund request form and have been approved, your health insurance coverage will terminate on 06/30/2023.

To qualify for a refund, there are several requirements that you must meet.

Required Qualifications:


  *   You MUST be returning to your home country and provide all required documentation (Provide a copy of a purchased plane ticket with itinerary- not just itinerary), that shows that you will be returning to your home country in May or June).
  *   Health Insurance must be paid in full ($2,013).
  *   You must obtain a signature of school official confirming completion of your degree program and that you will be graduating in May 2023.
  *   Provide a US mailing Address.
  *   Complete and submit the form by May 31st, 2023.


The refund Request form is Due: May 31st, 2023.



Once you have completed the form, please email us the form at: intlinsurance at tamuc.edu<mailto:intlinsurance at tamuc.edu>. We will then forward it to Academic Health Plans for Approval.
