[Intl-stu] Students applying for OPT and OPT extension during fall 2010

[International Students]

If you have an appointment with the ISSO for OPT or OPT extension processing please come prepared.

 

ALL DOCUMENTS SHOULD BE FILLED OUT AND SIGNED PRIOR TO YOUR APPOINTMENT.  We DO NOT require students to wait to fill out and sign the documents in person at the appointment.  Please fill our all forms and sign them where required prior to your appointment.    

 

·         The I-765 and I-538 must be filled out completely and signed prior to your appointment.  

·         The money order must be made out to Dept. of Homeland Security prior to your appointment and most money orders have a place to write your name which is strongly recommended.  

·         The two passport photos must have your 11 digit I-94 number and full name written on the back.  

·         You must provide your current physical address, personal phone number, and email on a separate sheet of paper (regular 8 ½ by 11 sheet of paper).

Failure to have all documents completed and signed will result in your appointment being rescheduled at a later date and time.
