Looks fine to me, Hannah, although it seems to lend itself to self-nomination. I'm not sure how someone would use it to nominate someone else . . .

Subject: [CBW_Exec] CBW Exec Nominations Form

Hello Exec Board:

Liz and I have tried to take the next step in the conversations we've been having about how to expand, update and invigorate the Exec Board. We'd like to create a nomination form. See draft below and see if you have any comments. Please let us know by week's end (Feb 17) as we would like to send this out to the list next week, looking for nominations and self-nominations. (This would be a nice readable form format; it is pasted in here for ease of access).
