[VPAAEtal] Revised Admission to Graduate School Procedure

President Dan Jones recently approved the revised university procedure
11.04.99.R0.01 Admission to Graduate School

Other than some reformatting and renumbering, the only change in the
procedure is in section 4.2-"probationary" has been changed to
"conditional."
