The Academic Practices Committee of the Faculty Senate is seeking faculty
opinions regarding a 

potential schedule for a mid-semester break during the fall semester
(mid-October).  The Faculty

Senate is hoping to propose the idea to the administration and would like to
know what the faculty 

preference is for the potential break.  Please check which of the following
options you prefer and return
