[TACRAO] Listserv Opt-Out of Directory Information

UT Tyler requires in-person submission of our Directory Information Non-Disclosure Form with a valid photo ID to opt out of Directory Information. As noted on the form itself, any other submission method must be approved by the Office of the Registrar. The form remains valid until rescinded, which must be done via submission of a second copy of the form with the appropriate option selected.

-Does your institution offer an online process for students to Opt-Out of Directory Information?
If yes:    What documentation is collected/recorded on the student record of request to opt-out?
                              Can they revoke/rescind restriction of directory information online as well?


-Are students required to submit an opt-out request every term or does the restriction stay active until student rescinds it?
