POSITION SUMMARY:
The Case Manager is responsible for planning and coordinating activities related to discharge planning.  Developing and implementing a plan of care designed to meet the patient's post hospitalization needs while ensuring continuity of care. The CM contacts external case managers/managed care organizations for certification and recertification of insurance benefits throughout the patient's stay, and will assist the treatment team in understanding the insurance company's requirements for continued stay and discharge planning. The Case Manager is responsible for having a thorough understanding of the patient's treatment through communication with the treatment team.  The CM advocates for the patient's access to services during treatment team meetings and through individual physician contact.
No supervisory function.

QUALIFICATIONS:
Education:    Master's Degree from an accredited college or university in social work required
Experience: Minimum of one (1) year experience in related disciplines with hospital experience strongly preferred.
License:   LMSW Required.
Additional Requirements: CPR & Handle Care training within 30 days of employment and prior to assisting in a restraining procedure.  May be required to work flexible hours and overtime.
