Job Description:

	
The Diagnostic Patient Navigator serves as a point of contact for
physicians, patients, and caregivers to provide resources and assistance
with accessing oncologic diagnostic services offered within CHRISTUS St.
Michael Health System and in the community. This position is an advocate
for the patient in their dealings with the healthcare community up to
the point of diagnosis, assisting the patient as needed throughout the
diagnostic process and connecting them to the proper providers and
resources in the continuum of their care. 

 

Job Requirements:

	
High school diploma or equivalent. Active Texas Licensed BSN or LMSW.
Cancer specific experience is a plus. Experience in breast
cancer/women's health is a plus. Requires problem solving, decision
making and critical thinking. Requires excellent leadership,
organizational, written, and verbal communication and excellent
interpersonal skill. Must be able to work in a self-directed environment
with an ability to work in a team environment. Excellent presentation
skills. Computer literate; Microsoft Office competency required.
Ability to communicate effectively, both verbally and in writing.
