Hospice medical social worker needed  to provide basic psychosocial
assessment and care to hospice patients and their caregivers.  Hospice
medical social worker is a member of interdisciplinary team and provides
patient care utilizing plan of care as developed by team.  Hospice
social workers initiate appropriate referrals and assist eligible
patients in securing needed benefits and assistance, provide appropriate
counseling to patients and caregivers and lead /facilitate
patient/caregiver support groups, as well as other social work duties.

Home Hospice provides comprehensive training in hospice social work
