Phoenix Hospice, based in Arlington, TX, has an immediate opening for a Hospice LMSW Case Manager in the Tarrant County/western Dallas County area.  Must have a master's degree in social work from an accredited college or university with a minimum of one to two years social work experience, with previous hospice or homecare experience preferred.  Reliable transportation, current driver’s license and current auto insurance also needed.  The successful candidate will have a specific passion for hospice work, and a desire to be a stable part of a strong hospice team committed to excellent client care.
