Description


Responsible for assessing the patients' and families' psychosocial,
community service, IDG and economical needs, and working to meet these
needs. The Social Worker is also the key contact in assisting the
patinet/family in the development of the advance directives. 


Qualifications


Master's degree in Social Work preferred. Licensure where required. 
Minimum of one to two years experience as a social worker. 
Previous hospice or home health care experience preferred. 
Must have reliable transportation, current drivers license and current auto
insurance.
