remember that the form <b>MUST</b> include (<b>and cannot be processed without</b>) your signature as well as your last date of attendance. If your class was an online class, this would be the last date that you logged in
<b>AND</b> participated. <span style="color:#1F497D">Please remember that Summer I and Summer II are in different semesters. Summer I is from 06/05/17 – 07/06/17. Summer II is from 07/10/17 – 08/10/17.
