[Students] Student Disability Resources and Services: Request for Spring 2010 Accommodations

If you are registered with Student Disability Resources and Services and
receive classroom accommodations and have not submitted a request for
instructor accommodation letters for the Spring 2010 semester, please
complete the attached form and send to Student Disability Resources and
Services (email, fax, mail, or drop off).  Please remember that
accommodations are not retroactive.

 

If you have submitted the request form and been notified that your
letters are complete, please stop by our office to pick up your letters
and deliver to your instructors.
