911 use, collaboration with community partners and city departments on how to provide complete and effective care to patients to maximize health and safety.

<p class="MsoNormal"><span style="font-size:12.0pt;font-family:"Times New Roman",serif">City of DeSoto- hiring an LMSW who will provide intensive behavioral health case management to citizens in the community who require support to obtain and maintain mental
health services and treatment. This will require home visits, follow ups, psychoeducation and performing of complete biopsychosocial assessments to determine barriers to engaging in care. This position will work with the new behavioral health care team through
DeSoto Police. <o:p></o:p></span></p>
<p class="MsoNormal"><span style="font-size:12.0pt;font-family:"Times New Roman",serif"><o:p> </o:p></span></p>
<p class="MsoNormal"><span style="font-size:12.0pt;font-family:"Times New Roman",serif">City of Mesquite- LCSW to provide psych assessments in active 911 calls, recommend levels of care and assist clients in gaining and maintaining mental health treatment.
They will also help in homeless outreach and resource connection. <o:p></o:p></span></p>
