CC Young Senior Living is hiring a long term care social work  manager who is in charge of social services for those patients who are not on hospice care.   The job would consist of filling out intake/initial assessment forms for newly admitted patients, resourcing for patient's needs, making room visits to see patients, etc. Those interested in applying need to have at least an LBSW.
