Incentives

By participating in the study your ID number will be entered in a random drawing by all participants to receive one (1) $20 gift certificate to any Wal-Mart store.

Protection of Privacy and Confidentiality

You will be assigned an ID number to login to the Qualtrics website in order to keep your confidentiality of records and not identifying your participant in the study. We will do everything we can to protect your privacy and confidentiality. We will not tell anybody outside of the research team that you were in this study or what information we collected about you in particular.

The data will be stored on the Qualtrics website and then when downloaded the information will be stored in Benjamin May's office in a locked cabinet for the duration of the study and after 3 years the data will be destroyed by Benjamin May. Only the research team of Benjamin May and Linda Openshaw (Co-Investigator) will have access to all data to ensure confidentiality.

Choosing to Be in the Study

You do not have to be in this study. You may choose not to take part and you may choose to stop taking part at any time. Participation is voluntary.
