Do not retype the form in another format.
Registration requests will only be accepted in this format.

The attached revised form includes new evening sections for SWK 505 & 506 in MPLX.
Please note they are not on the same day!!!

Also note that SWK 554 and SWK 595 in the MPLX are full. No other sections will be added.

If a new request: Save it as an .XLS document Name it: Registration Request: (your name) or

If a Revised request, Save it as an .XLS document Name it: Request Revised: (your name).

You'll be unable to type the information in the long boxes.  The information must be selected from the drop down list.  Please do not alter the form in any way.

Once you have selected your courses and are ready to submit the information, please save the form and send it as an email attachment to me at janis.cirkles at tamuc.edu<mailto:janis.cirkles at tamuc.edu>.

Registration requests will be worked in order received. If you previously submitted a request and choose to submit a revised request it will be worked in order from the latest received date shown. Once the permits have been issued, I'll send you an email to let you know that you can register.

Also, please double check your form before submitting and be sure that you have selected the correct courses, i.e., correct date, time, and location, to avoid unnecessary delays.
