Full time position (possibly two) for busy Garland, Texas nursing home
Social Services Department. 

 

Looking for LBSW or LMSW for basically 40 hours a week Monday - Friday,
8am - 5pm.

 

Must be licensed and able to sign off on assessments and social worker
notes.  

 

The position will require:

Interviewing skills and techniques and listening skills.

Ability to input data into a computer as well as hand write notes in
chart.

Communication with medical staff, residents, families and responsible
parties via email and telephone conversation as well as in person.

Time management skills along with the ability to schedule and track
assessments and documentation required for compliance to Medicare
,Medicaid, State of Texas policies and regulations and the policies of
the facility and to meet required deadlines for this documentation.

Discharge planning of residents and transfer planning from rehab to home
or assisted living.

 

Salary commiserate with experience and knowledge of chronically ill and
geriatric population as well as mental health issues. 

 

Potential for two positions to be filled to divide the social work
duties and responsibilities of this large (4 unit) facility.
