Creative Solutions in Health Care is currently seeking Social Workers
for our facilities.  The Social Worker is responsible for developing
social histories of residents as well as a social assessments and care
plans for each resident. 

Job Description 

*	Participates in interdisciplinary care plan conferences for
residents; 
*	Designs and develops social service programs that meet medically
related social and emotional needs of residents and complies with
regulatory guidelines;

Requirements 

*	Bachelors Degree in social work (BSW) 
*	Internship in a long term care facility a plus, but not
required.
