At Pate Rehab our Case Managers are an intricate part of our
multi-disciplinary treatment teams in achieving successful outcomes for
our patients. This position requires knowledge of working with funding
sources in order to maximize rehab opportunity, ability to negotiate
funding, community resources and discharge planning. Strong
interpersonal, communication and computer skills are a must. Ability to
multi-task and insurance knowledge is very helpful. 

 

Requirements:

Current state license.  

A  bachelors/masters degree in social worker
