<b>STUDENTS WITH AN APPROVED WAIVER FOR FALL 2018 WILL NEED TO SUBMIT A WAIVER REQUEST FOR SPRING 2019.

<p class="MsoNormal"><b><span style="font-size:14.0pt;background:yellow;mso-highlight:yellow">The SSHIP premium amount for the Spring 2019 semester is $1662.</span></b><span style="font-size:14.0pt"> This charge will be added to student accounts after a student
has registered for courses for Spring 2019 and once the university begins applying tuition and fees. Payment will need to be made by the 12<sup>th</sup> day of class or Student Accounts will assess a late fee penalty for students who have not paid the SSHIP
premium. The coverage period for students will be January 1, 2019 to August 31, 2019.
