PLEASE REVIEW ATTACHMENT FOR ATTESTATION FORM. COMPLETE AND SIGN ATTESTATION FORM BEFORE UPLOADING TO PORTAL. THANK YOU.

Dear International Students,

In order to be considered for a waiver of the Spring 2021 SHIP Premium, you must submit a formal request of waiver by using the AHP Waiver Portal:   http://www2.academichealthplans.com/school/22212.html and attach the following required documents:

1.            I 94 travel information showing proof that you have left the US.
Go to https://i94.cbp.dhs.gov/I94/ .  In the box, "Need a History of Your Arrivals & Departures?  Click the Blue Box "View Travel History". Save as a PDF to upload.
2.            Completed and Signed Attestation Form

The portal opens Wednesday, January 13th, 2021 for 2 weeks only. The deadline to apply for the COVID-19 Waiver is Tuesday, January 26th, 2021 (11:59 pm CST). If you fail to apply for the waiver through this portal, you will be charged for Spring 2021 and the charge will remain. If you have friends who are registered for Fall 2020 classes, check with them to be sure everyone hears and apply for the waiver, if they are currently studying outside of the U.S. for Spring 2021. This waiver is NOT for students who are currently in the U.S. Spread the word!
