Berry Family Services has a job opportunity for a Community Support
Attendant . The client lives in  Lewisville and is a  22 year old female
with a diagnosis of severe idd.

The client needs someone to transport her from her home in Lewisville to
the Day Hab 15 miles from their home 5 days a week for a total of 7.5
hours a week.  

The attendant would need to arrive at the home at 7:30 am and have her
at Day Hab by 9am. 

The pay is $17.50/hour for community support on a W-9 (taxes must be
paid by the individual at the end of the year).

The requirement is for the attendant to be 18-years-old or older, with
reliable transportation, auto insurance and a clear criminal record.
