[bsw] Petition for Course Overload Request

Good morning,

 

We've had a several inquiries this morning regarding overloads.  Just a
reminder, the procedure for overload forms was changed. Students must
complete a typed overload form and have it signed at the department
level. 

Please see attached PDF file. Make sure you have   available to ensure
all class information typed on overload form is correct.

 

Type information into form and print it. Adobe acrobat will not allow
you to save form. You can scan/email, fax or bring completed signed form
to Henderson 311 for department signature. I'll then forward to Dean's
office for final approval. Thank you for your patience and understanding
during this transition.
