[bsw] Petition for Course Overload Request

Good morning,

 

We've had a several inquiries this morning regarding overloads PDF file.
Apparently, it's not letting you type information into form correctly.
I've attached a Word Document file for your use. 

 

Students must type overload form, handwritten forms are not accepted,
print out completed form, sign it then forward it to the Social Work
department for signature. 

 

You can scan/email, fax or bring completed signed form to Henderson 311
for department signature. I'll then forward to Dean's office for final
approval.
