Description


 

The Social Worker will assist in the facilitation of care across the
continuum for the patient populations. Evaluation of psychosocial needs,
crisis intervention, and timely and appropriate discharge planning are
key components for this position. The ability to collaborate with case
managers, physicians, staff, families, and community referral bases to
enchance optimal patient outcomes is essential. 

Shift is 10am to 10pm, three days per week, rotates Saturdays.

  


Qualifications


 

*	Bachelor's or Master's degree in Social Work. 
*	Current licensure by the Texas State Board of Social Work
Examiners. MSW Preferred.
*	BLS required from American Heart Association.
*	Inpatient experience working with all patient populations is
desirable.
*	Two years hospital experience strongly preferred.
